Medical Services Guidelines

Welcome to the PUPT Medical Services page. Here you will find all the necessary information regarding the medical services available to you.

Step 1: SAR-Form Submission

Complete the SAR-Form, Route & Approval Slip. Ensure all fields are accurately filled out.

Step 2: Medical Certificate

Submit a current medical certificate from any accredited healthcare facility. It must be on official letterhead and signed by a licensed medical doctor (include printed name, signature, and license number).

Step 3: Chest X-ray

Provide a chest x-ray (PA view) taken within the last 6 months. Submit the original copy along with the plate.

Step 4: COVID Vaccination Card

For vaccinated applicants, submit a copy of your COVID vaccination card. This is required for record-keeping purposes only.

Step 5: Health Information Form

Complete the Health Information Form for Students, downloadable from pup.edu.ph, and attach a 2x2 ID picture. For students under 18 years old, ensure the form is signed by a parent or guardian.

Step 6: Document Copies

Submit one photocopy each of the documents from steps 2 through 4.
